How to request services/accommodations:

The following are steps for students to request disability services or accommodations at CSU Monterey Bay:

1. Contact the office for Student Disability Resources (SDR) to request a Prospective Student Packet, or download its contents from the links below or on the SDR Forms Page.
    
2. Complete and submit the SDR Needs Assessment Form.
    
3. Submit to SDR Disability Documentation/Verification from a licensed physician or qualified specialist. Two forms are available for Documentation/Verification of Disability: The Disability Documentation form is intended for students with Psychological and/or Learning Disabilities (Includes ADHD/ADD). The Disability Verification form is intended for all other students with disabilities. (Examples: Mobility, Vision, Hearing, Chronic Illness, etc.)
    
4. Schedule an appointment with a Disability Resources Advisor to determine how SDR can best serve you. Appointments can be scheduled in-office or by telephone at (831) 582-3672 (Voice), or 582-4024 (TTY).
